Hockey Registration Dates are as follows:

Thursday, September 9^th    6:00 to 8:30 PM at Polar Arena in the Mezzanine

Sunday, September 12^th     3:00 to 6:00 PM at the NSP Community Center

Monday, October 4th     6:00 to 8:00 PM at Polar Arena – Late registration for new players only.  

*****IMPORTANT REMINDER*****

All players must register with USA Hockey before registering with NSPAHA.

Ø  Visit www.usahockey.com and click on the membership link then the register a player link.

Ø  Select the Ice Players & Coaches tab and follow the instructions.

Ø  Pay the $40 registration fee with a credit card or Visa gift card.

Ø  Print the receipt with bar code and bring it to one of the registration sessions above.

What else do I need to bring with me?

All players need to bring with them

Ø  3 Checks... 1 for volunteer hours , 1 for the fundraiser, 1 for NSP registration fee

Ø  Self-Addressed Stamped Envelope to return your Volunteer Check (if you do not bring an envelope your check will be shredded when your volunteer hours have been fulfilled)

Ø USA Hockey confirmation sheet with bar code (register online at www.usahockey.com)

For new players or any player not registered with NSPAHA for the 2009-2010 season, please also bring:     

Ø  A copy of your government issued birth certificate.

Ø  The name, address and policy number of your health insurance company.

Ø  The name and phone number of your doctor/clinic and your preferred hospital.
