USA Hockey Step by Step guide to Online Registration

North St. Paul Area Hockey Association Summer 2011
Hockey Registration Dates are as follows:
Wednesday, July 20th 7:00 to 9:00 PM at Tartan's Blue Line Room
Wednesday, August 24th 7:00 to 9:00 PM at Tartan's Blue Line Room
Wednesday, September 14th 7:00 to 9:00 PM at Polar Arena in the Mezzanine
Saturday, September 17th 1:00 to 4:00 PM at Polar Arena in the Mezzanine
All players must register with USA Hockey before registering with NSPAHA.
Ø Visit www.usahockey.com and click on the membership link then the register a player link.
Ø Select the Ice Players & Coaches tab and follow the instructions.
Ø Pay the $40 registration fee with a credit card or Visa gift card.
Ø Print the receipt with bar code and bring it to one of the registration sessions above.
What else do I need to bring with me?
All players need to bring with them
Ø 3 Checks... 1 for volunteer hours , 1 for the fundraiser, 1 for NSP registration fee (if you choose multiple payments you will need more than one check)
Ø Self-Addressed Stamped Envelope to return your Volunteer Check (if you do not bring an envelope your check will be shredded when your volunteer hours have been fulfilled)
Ø USA Hockey confirmation sheet with bar code (register online at www.usahockey.com)
For new players or any player not registered with NSPAHA for the 2011-2012 season, please also bring:
Ø A copy of your government issued birth certificate.
Ø The name, address and policy number of your health insurance company.
Ø The name and phone number of your doctor/clinic and your preferred hospital.
